But it was Neerja, which released in the year 2016 and is Ram Madhvani's baby, that marked his entry into the big league. It traces the life of real-life brave woman Neerja, an air hostess, who sacrificed her life but saved millions of lives due to her presence of mind and alertness in a hijack attack.

Since Ram and Mitesh had previously worked with each other on TV commercials, he decides to sign Mitesh for the movie. However, he confessed being confused about his decision initially but could overcome it after seeing Mitesh's confidence and work. The makers shot the entire movie inside the confines of a commercial plane which made it a challenge for Mitesh to work on as he had to work using the multi-camera setup in a cramped place with more than fifty artists. Also, he shot the film without using any lights and had to rely on only the natural brightness of the aircraft.

The movie, as well as his work, received critical success as well as the viewer's love. In fact, he also won the Filmfare Award for Best Cinematography for Neerja despite having competent competitors in the category.
